OUR SERVICES

As a subsidiary of a GP Federation that are an integral element of a wave 1 neighbourhood, PCA has first-hand experience of working to support newly emerging neighbourhoods.

Primary care data has always been key to supporting Herefordshire General Practice to truly understand themselves, their contribution to the system and their communities. With tools such as Valuing General Practice to provide oversight on the county’s total appointment activity (including EA & OOH), Health Inequalities which has identified areas for improvement to benefit the population, as well as our Population Health + dashboard which has enabled an increased understanding of the health of the county and identification of cohorts of patients for preventative interventions based on demography, long-term conditions and deprivation; general practice prides itself on making data driven interventions.

We’re now working to tailor more outputs to the needs of the neighbourhood, consolidating data from multiple parts of the health and care system, to ensure that there is a clear sight of the progress and impact made.

If you would like to discuss potential support for your neighbourhood, either as part of a confirmed wave 1 site, or an aspiring second wave, then drop the team a message here: contact us.
