Our PHM+ Dashboard is a flexible, yet powerful tool, helping you to better understand your population, identify inequalities and select cohorts for targeted interventions.

With millions of data points, which can be analysed within a couple of clicks, you can filter between a range of factors including

  • Demography
  • Deprivation
  • Lifestyle factors – such as obesity, smoking and homelessness
  • Long-term conditions – by number of LTCs or specific condition
  • Medical Coding, and
  • Contacts – including GP consultations and appointments, A&E Attendance and Hospital Admissions

We’ve used this tool with practices, PCNs and place to identify cohorts for proactive work, based on variables such as LTCs admissions and frailty, challenge access inequalities and monitor impacts.
