THE 10 YEAR PLAN
The start of July saw the release of the 10 Year Plan. Our initial thoughts are on our website here.
Everyone has now had an extended time to digest this and think a little more about how they might play a part in the neighbourhood delivery model, and places are being asked to make their application as the early adopters of the NHHIP.
Whether you see yourselves as a Multi Neighbourhood Provider, or a Single Neighbourhood Provider, the case remains the same, you will need to understand your primary care data, in order to play a crucial role in shaping their development.
For GP at scale – Federations and Collaboratives – seeking to lead local neighbourhood developments, but have to date not invested in their analytics, you may need help getting up and running – building an understanding of local delivery, local services, and importantly, local population, at pace.
Similarly, we see Trusts that are keen to lead developments and whilst they have analytics teams in place, they often have a limited understanding and experience of general practice data. Without this insight, understanding and analytical input, it might be hard to bring local general practice along on the journey.

OUR PHM WEBINAR WITH REDMOOR HEALTH
Dr Lauren Parry and Paul Harris joined forces with Clare Temple from Redmoor Health & Jeremy Martin, to talk you through some examples of how we’ve deployed our PHM+ tool locally in Herefordshire.
Using this tool, we have been able to lead conversations with partners in the county using general practice data to risk stratify, and pick out cohorts of the population to target for interventions, with the view of reducing A&E attendances. Added to this, there are also examples of how this tool has been used to reduce health inequalities, in areas which could easily have been missed.
Using coded medical data, as well as the lifestyle data captured in EMIS, Herefordshire is able to find manageable cohort numbers to proactively engage with.

COMMUNITY PHARMACY
Within the 10 Year Plan, there is a clear expectation of a greater role for Community Pharmacy. But do you know how effective your local area is in Pharmacy First and other contracts. PCA’s Community Pharmacy dashboard is a cost-effective approach to collating your contractual data into a single place to better view the local position.
Whether you’re an ICB, an LPC or a PCN, imagine aligning this data alongside your local PHM data, to add a new level of insight into how local communities are engaging with healthcare. All of a sudden, you can start to build up the understanding of neighbourhood resilience.
