PHM IN ACTION
Dr Lauren Parry and Paul Harris joined forces with Clare Temple from Redmoor Health & Jeremy Martin, to talk you through some examples of how we’ve deployed our PHM+ tool locally in Herefordshire.
Using this tool, we have been able to lead conversations with partners in the county, using general practice data to risk stratify, and pick out cohorts of the population to target for interventions, with the view of reducing A&E attendances. Using coded medical data, as well as the lifestyle data captured in EMIS, Herefordshire is able to find manageable cohort numbers to proactively engage with.
